Bacteriophages in urologists: experience of use

The drug Pyobacteriophage polyvalent purified can be used for the prevention and treatment of infectious and inflammatory complications in percutaneous nephrolithotripsy as an alternative to antibiotics.

In February 2020, a scientific and practical conference with international participation , "Rational Pharmacotherapy in Urology – 2020," was held in Moscow. Professor Tamara Sergeevna Perepanova, MD, Head of Department at the N.A. Lopatkin Research Institute of Urology and Interventional Radiology, Chair of the Moscow Society of Urologists, Chair of the International Public Organization "Rational Pharmacotherapy in Urology," and Board Member of the European Section of Infections in Urology of the European Association of Urologists, delivered a keynote address on clinical guidelines for antimicrobial therapy and the prevention of kidney, urinary tract, and male genital infections. Professor Perepanova noted that the increasing antibiotic resistance of uropathogens in both community and hospital settings remains a significant issue in urology. The search for alternative means of preventing and treating urinary tract infections continues. The speaker presented the results of a study on the effectiveness of using purified polyvalent pyobacteriophage for the treatment and prevention of infectious complications after percutaneous nephrolithotripsy in comparison with antibiotic prophylaxis.

Before percutaneous nephrolithotripsy, 30 patients received pyobacteriophage, and another 30 patients received ciprofloxacin. A comparative analysis revealed a reduced incidence of systemic inflammatory response syndrome in the bacteriophage group compared to the antibiotic group, with an equal treatment and prophylactic effect on the seventh postoperative day. Therefore, purified polyvalent pyobacteriophage can be used for the prevention and treatment of infectious and inflammatory complications following percutaneous nephrolithotripsy as an alternative to antibiotics. To improve antimicrobial activity, continuous monitoring of microorganism susceptibility to bacteriophage preparations and regular updating of bacteriophage preparations for pathogens specific to a particular hospital or patient are necessary.
